Performer Details Form

Please fill in the below form with the required information for each performer (under and over 18 years of age). It is very important we have the correct information about any allergies, ongoing medical conditions, dietary requirements and, for children, parental consent to be photographed and recorded. 
Please note that we will be filming and have arranged for a professional photographer to take pictures during the Dress Rehearsal and the Shows. We will not be able to exclude any performer from the footage, however, when it comes to sharing the images and videos, we will make sure to cover the faces of any performing children for whom we do not have parental consent to share with others or post on social media.
